It’s not always the easiest decision, but long-term care is often the best option for you or your loved one who can no longer be cared for at home due to illness, injury, frailty or other limitations. Long-term care is determined by a healthcare professional who assesses an individual’s ability to perform routine tasks of life. With the guidance of Dr. James Larsen and Dr. Heith Waddell, long-term care is provided Crook County Medical Services District's long-term care facility by licensed nurses, licensed practical nursing and certified nursing assistants aide in a full interdisciplinary care team.


Crook County Medical Service District's long-term care includes a variety of services for people who cannot care for themselves for extended lengths of time. If you or your loved one can no longer be cared for in the home, long-term care offers around the clock care by a specially trained team of healthcare professionals. 


Crook County Medical Services District will create an individualized care program for your or your loved one. In the initial meeting with our trained staff, we will put together a care plan that ensures no stress for you or your loved one. Many activities, social events, meals, etc. are provided at our Long Term Care facility.
